Crystal Lake RV Resort Reservation Form

To begin the reservation process...

1. Complete and submit the following form.
2. You will be contacted by email to confirm your reservation.
3. A 30% deposit by check is required within 15 days of confirmation to guarantee your reservation of a site.
   
4. The balance of your contract amountis due upon arrival. Checks are the prefferred method of payment. But we also accept cash, Visa and MasterCard. Make checks payable to: Crystal Lake Realty, LLC. Please mail them to:

5. All credit card payments will have a 3% processing fee added to the total at the time of arrival.
6. Cancellation notice of 15 days prior to arrival is required. Your prepayment will be refunded less a 20% processing fee (minimum $50).
7. ALL ARRIVALS AND CHECK-IN DECEMBER-APRIL MUST BE MADE DURING RENTAL OFFICE HOURS MONDAY-FRIDAY 9am to 4pm       SAT-SUN 11am to 3pm
   SPECIAL SITUATION CHECK-IN ARRANGEMENTS MUST BE MADE IN ADVANCE WITH OFFICE!

ALL RV VEHICLES AND TRAILERS, ARE
REQUIRED TO  HAVE PADS FOR THEIR "JACKS"
